Neuropathie door Metformine

Langdurig Metformine gebruik kan indirect de oorzaak zijn van een beginnende of verergerende neuropathie. En wel een vitamine B12 deficiëntie polyneuropathie.

• Of: Long-term use of metformin is associated with malabsorption of vitamin B(12) (cobalamin; Cbl) and elevated homocysteine (Hcy) and methylmalonic acid (MMA) levels, which may have deleterious effects on peripheral nerves. [1]

In een recente studie (2009) bleek dat diabetes patienten die langdurig Metformine gebruikten inderdaad lagere vitamine B12 gehaltes in het bloed hadden.

ZIj hadden ook en een ernstiger neuropathie dan de patiënten die geen Metformine gebruikten. De aanbeveling was dan ook om regelmatig de vitamine B12 gehaltes te laten checken:

• Metformin exposure may be an iatrogenic cause for exacerbation of peripheral neuropathy in patients with type 2 diabetes. Interval screening for Cbl deficiency and systemic Cbl therapy should be considered upon initiation of, as well as during, metformin therapy to detect potential secondary causes of worsening peripheral neuropathy. 

Een gevalsbeschrijving van Metformine neuropathie

In een recente gevalsbeschrijving wordt eerst gesteld dat in 30% van alle patiënten die met Metformine behandeld worden, vitamine B12 deficiëntie voorkomt. Dat tekort zou ontstaan na langdurig gebruik. De auteur waarschuwt voor de misdiagnose dat met om een diabetes neuropathie zou gaan:

• Vitamin B12 deficiency, which may present without anemia and as a peripheral neuropathy, is often misdiagnosed as diabetic neuropathy, although the clinical findings are usually different.

Hij wijst er bovendien op dat het van belang is om tijdig vitamine B12 te geven, omdat als de B12 deficiëntie eenmaal tot neuropathie heeft geleid, je dat niet meer weg krijgt met B12 suppletie:

• Failure to diagnose the cause of the neuropathy will result in progression of central and/or peripheral neuronal damage which can be arrested but not reversed with vitamin B12 replacement.  [2]
