HSAN, 5 typen van erfelijke neuropathie
HSAN: Er zijn 5 verschillende typen van de erfelijke sensibele en autonome neuropathie, de Hereditary Sensory Autonomic Neuropathy (HSAN). De verschillende soorten van HSAN zijn zeldzaam.
Naast de Familiare Amyloid Polyneuropathie, is de erfelijke sensibele en autonome neuropathie een voorbeeld daarvan. Andere vormen zijn de ziekte van Fabry en de Porphyrie. Bij al die ziektes komen ontregelingen voor van het autonome zenuwstelsel.
HSAN Hereditaire Sensorische en Autonome erfelijke Neuropathie
De Hereditaire Sensorische en Autonome Neuropathieen (HSAN), worden ook wel aangeduid met de term hereditaire sensorische neuropathieen; (HSN) als er geen autonome stoornissen deel uit maken van het ziektebeeld. Het gaat bij HSAN om een groep met oorzaken op genetisch gebied.
Er worden verschillende soorten ziektebeelden onderscheiden, allen zeldzaam. Bijvoorbeeld de HSAN1 is een dominant overervende sensomotorische axonale neuropathie die rond het 10-20ste levensjaar zich manifesteert. Deze variant wordt veroorzaakt door een mutatie van het SPTLC1 gen (605712) op chromosoom 9q22.
Verder onderscheiden we:
- HSAN2 (201300) die ontstaan is door een mutatie in de HSN2 isoform van het WNK1 gen (605232) op chromosoom 12p13,
- HSAN3 variant (223900) veroorzaakt door een mutatie in het IKBKAP gen (603722) op het chromosoom 9q31,
- HSAN4 (256800) door een mutatie van het NTRK1 gen (191315) op chromosoom 1q21,
- HSAN5 (608654) door een mutatie op het NGFB gen (162030) op 1p13.
De variant van HSAN die gekarakteriseerd wordt door een reukstoornis, namelijk anosmia (608720) wordt gezien als een specifieke variant van HSAN.

HSAN type 1 kenmerkt zich door een afwezigheid van pijn en temperatuur gevoel door de degeneratie van de dunne vezels:

4- Deoxy-sphingoid bases (DSB): 1-deoxy-sphinganine & 1-deoxymethyl-sphinganine
- DSBs have neurotoxic effects on neurite formation in vitro
- Clinical features
- Onset: 2nd decade or later; Average 25 years
- Distribution: Distal > proximal; Symmetric; Legs > Arms sensory, autonomic & reflex loss

- Loss: Pain & Temperature (Small fiber); Large fiber loss also occurs
- Spontaneous sensations: Paresthesias are Rare; Lancinating pains in some kindreds; Burning pain in some
- Charcot’s joints (Neurogenic osteoarthropathy)
- Progression: Succession of exacerbations
- Location: Feet, Severe mutilation & shortening; Occasional hands, Thickened fingers
- X-rays: Distal demineralization; Metatarsal tapering (Licked candy-stick)
